All based on context. Here, you see two versions of PURSE. One is held by the handle, the other is a 'clutch,' and can be held under the arm (armpit), so it is signed that way. The Non-Dom arm does NOT have to move up as The Man in the Blue Shirt does. I suppose Deaf women can sign more easily when they don't have to hold a purse in their hand...!
